About the Position

The Hospice Acute CNA is responsible for providing high-quality, compassionate care to terminally ill patients. The CNA works under the supervision of a registered nurse (RN) and as part of an interdisciplinary team to meet the physical, emotional, and personal needs of patients in the final stages of life.

JOB DUTIES

  •  Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, feeding, and   toileting.
  •  Provide basic nursing care, such as taking vital signs (temperature, pulse, respiration, blood pressure) and reporting significant changes to the supervising RN.
  •  Assist with mobility, including turning and positioning patients to prevent bedsores, and helping with walking or transferring between bed and chair.
  • Administer personal care and hygiene, including oral care, skin care, and hair care.
  • Provide compassionate support to patients and their families, respecting their emotional and spiritual needs.
  • Accurately document all care provided, including observations of patient condition, in accordance with hospice policies and regulatory requirements.
  • Communicate effectively with the RN and interdisciplinary team about changes in patient status, pain levels, and any concerns that arise.
  • Ensure timely and accurate reporting of patient information to appropriate team members.
  • Work closely with the hospice team, including nurses, social workers, chaplains, and volunteers, to provide comprehensive care.
  • Participate in team meetings and contribute to care planning discussions
  • Adhere to all hospice policies and procedures, as well as state and federal regulations.
  • Practice standard precautions and infection control techniques to ensure the safety of patients and staff.
  • Recognize and respond appropriately to emergencies, initiating life-saving measures as needed and as within the scope of CNA practice.

Job Requirements

  • High school diploma or equivalent.
  • Current certification as a Certified Nursing Assistant (CNA) in the state of employment.
  • Prior experience in hospice, palliative care, or acute care settings is preferred.
  • Strong interpersonal and communication skills.
  • Ability to provide care with compassion, dignity, and respect.
  • Proficiency in basic nursing care and the ability to follow a care plan.
  • Ability to work independently and as part of a team.
