About the Position

The Palliative Care Social Worker will provide in-person psychosocial support and social work interventions to referred persons primarily in local hospitals and also on Goodwin Living campus locations/in the community, as necessary. This position is responsible for the utilization of health services to referred persons to include but will not be limited to: Home Health, Hospice, personal care services and rehabilitation/therapy services. This position requires travel throughout the Northern Virginia area.

Job Duties

  • Manages a caseload of Goodwin Living campus residents and patients of Goodwin Living Home and Community-Based service lines who are currently hospitalized; collaborates with residents/patients/families, hospital providers and Goodwin Living staff to complete all appropriate Goodwin Living and/or community referrals.
  • Facilitates goals of care discussions and provides direct social work interventions regarding chronic conditions, advance medical directives, code status, POLST forms, etc. and available resources.
  • Provides effective transition management between the hospital and Goodwin Living campuses/the community, as appropriate; serves as a consistent and direct point of contact for resident, families and hospital providers while hospitalized.
  • Participates in Goodwin Living interdisciplinary and campus meetings to provide and receive necessary information on referrals and resident updates.
  • Utilizes Goodwin Living and hospital electronic medical record systems, documents all necessary interactions according to applicable facility and regulatory requirements, tracks all necessary program data.

Qualifications

  • Master’s degree in social work from an accredited university; Licensed Clinical Social Worker (LCSW) in the Commonwealth of Virginia preferred.
  • Must be proficient in computer skills and able to learn at least two electronic medical record software.
  • Three (3) years of relevant experience in Social Work preferred.
  • Must possess ability to deal tactfully and effectively with patients, staff, family members and the public.
