About the Position
This PRN position involves providing skilled nursing care to patients in their homes during non-traditional hours, ensuring continuity and quality of care when regular staff are unavailable in compliance with all regulatory standards. In this position, the employee acts in reflection of Goodwin Home Health’s philosophy and mission of providing excellent standards of care as a full-time clinician in the field as well as performing supervisory tasks and additional duties such as meeting attendance and program development in conjunction with the nurse manager. The PRN after hours/on-call nurse needs to display the qualities of team leadership and mentorship to colleagues
Job Duties
- Provides direct nursing care in a Home Health setting under the guidelines of a physician’s order.
- Conduct comprehensive assessments of patients' health to identify immediate needs and provide appropriate interventions in support or modification of a patient specific Plan of Care.
- Respond to patient and family calls, offering guidance, education, and determining the necessity of in-person visits based on clinical judgment.
- Maintain accurate and timely clinical records, including evaluations, visit notes, care plans, and communication logs, in compliance per policy.
- Coordinate with the patient's primary care or specialty physician, case manager, and other healthcare professionals to ensure timely, cohesive, and comprehensive care.
- Perform wound care, manage chronic conditions, provide foley catheter and IV or line troubleshooting, and other nursing interventions as per the patient individualized care plan for urgent, on-call and after-hour needs.
- Provide immediate care in urgent situations, including arranging for transportation or hospitalization if necessary.
- Offer guidance to patients and families on managing health conditions, medication administration, and preventive care measures.
- Ability to commit to on-call shifts during evenings, nights, weekends, and holidays.
- Documents in patients' clinical records per Goodwin Home Health polices and standards
- Identifies patient needs and communicated them to the case manager (RN) in a timely manner
- Implements clinical care per Goodwin Home Health policies and procedures
- Implements and completes documentation of patient care and visits timely and accurately
- Attends mandatory meetings, seminars or any other requested in person department meetings as well as maintain 100% compliance with mandatory training, staff meetings at least 4 times per year
- Fulfills additional performance responsibilities as assigned and as necessary
- Reports changes in patient condition immediately to Clinical Nurse Manager or Administrator
- Immediately reports abuse, real or suspected to Clinical Nurse Manager, Administrator or DHP
- Maintains confidentiality of patient information as well as other staff members information at all times
- Promotes educations for patients, family members and caregivers
- Other duties assigned by Clinical Nurse Manager/Administrator
Job Requirements
- Graduate of an accredited school of nursing
- Legally and currently licensed by the state board of nursing to practice as a Licensed Practical Nurse or Registered Nurse in the Commonwealth of Virginia
- At least on year acute care experience, Home Health preferred
- Must have strong clinical assessment, critical thinking, and communication skills with ability to make independent decisions, work independently throughout the day and manage time effectively.
- Ability to commit to on-call shifts during evenings, nights, weekends, and holidays
- Current, unexpired CPR card
- Health clearance, PPD/Chest X-Ray
- Ability to communicate effectively in English, both verbally and written
- Must have knowledge of the duties and functions of Home Health care and its relationship to the healthcare community
- Maintain professional dress, demeanor and presentation at all times
- Must have the ability to move intermittently throughout the workday; lift, pull, push and move equipment, supplies, etc. up to 50 lbs. getting assistance when needed is a must
- Possess a Virginia Driver’s license, proof of automobile insurance and readily available, reliable transportation
- Must possess the ability to deal tactfully and compassionately with patients, family members and caregivers
- Must be willing to and able to participate in a teamwork approach, working with all disciplines involved in the patients’ care/case
- Possesses enthusiasm, flexibility, and the ability to adapt to the changing healthcare work environment.
