About the Position
The Full time pay per visit RN (Case Manager) is responsible for the care of patients assigned by Administrator/ Clinical Nurse Manager. The RN assigned will perform the admission assessment for that patient, identifying patient needs and status. The RN, under the physician’s nursing order, will create a Plan of Care to address all patient needs to provide optimal functioning
Job Duties
- Provides direct nursing care in a Home Health setting under the guidelines of a physician’s order
- Performs/completes OASIS documentation and ensures documentation is completed in a timely and effective manner; OASIS 24-48 hours, skilled visits 3-5 days.
- Performs a detailed assessment resulting in the development of a patient specific Plan of Care. Client clinical record documentation is done per agency policy
- Ability to maintain full time case load of 28-30 points per week by consistently communicating with Clinical Nurse Manager/ nurse scheduler to ensure productivity is met.
- Communicates with other team members regarding the Plan of Care and continues to work with team members for the duration of the patient’s case
- Effectively communicated with referring physician, either verbal or written, and provides accurate treatment notes and other necessary documentation
- Documents according to Home Health care policy, agency philosophy
- Prepares for patient discharge planning as soon as possible along with informing and preparing the patient, team members and physician
- On-going observational recording/reporting of changes in patients condition as soon as possible to the patients physician, the Clinical Nurse Manager and other team members
- Makes recommendations for involving other disciplines as deemed necessary
- Advises and consults with the patients family in the patients therapy program
- Provides supervisory visits of home health licensed practical nurses (LPN) and home health aides (HHA) to monitor for compliance with appropriate policies and procedures regarding patient care and rights
- Attends weekly IDT meetings and mandatory inservices; and maintains 100% compliance with Relias training and training for licensure.
- Documents all communication with other personnel to contribute to patient’s case conference notes
- Uses equipment and supplies effectively and efficiently and reports supplies to be ordered for patient care
- Maintains confidentiality of patient information as well as co-workers personal information if applicable at all times
- Implements infection control and safety measures as appropriate
- Consistently addresses patients concerns. Promotes education for patients, family members and caregivers
- Attends mandatory in-services and maintains 100% compliance with mandatory data based training
- Participation in meeting requirements for state health department survey of the Home Health Services is demonstrated
- Immediately reports abuse/suspected abuse to Clinical Nurse Manager, Director of Clinical Services or Administrator.
Job Requirements
- Graduate of an accredited school of nursing
- Legally and currently licensed by the state board of nursing to practice as a Licensed Practical Nurse or Registered Nurse in the Commonwealth of Virginia
- At least on year acute care experience, Home Health preferred
- Must have strong clinical assessment, critical thinking, and communication skills with ability to make independent decisions, work independently throughout the day and manage time effectively.
- Ability to commit to on-call shifts during evenings, nights, weekends, and holidays
- Current, unexpired CPR card
- Health clearance, PPD/Chest X-Ray
- Ability to communicate effectively in English, both verbally and written
- Must have knowledge of the duties and functions of Home Health care and its relationship to the healthcare community
- Must have great time management skills
- Maintain professional dress, demeanor and presentation at all times
- Must know and perform OASIS documentation
- Must have the ability to move intermittently throughout the workday; lift, pull, push and move equipment, supplies, etc. up to 50 lbs. getting assistance when needed is a must
- Possess a Virginia Driver’s license, proof of automobile insurance and readily available, reliable transportation
- Must possess the ability to deal tactfully and compassionately with patients, family members and caregivers
- Must be willing to and able to participate in a teamwork approach, working with all disciplines involved in the patients’ care/case
- Possesses enthusiasm, flexibility, and the ability to adapt to the changing healthcare work environment.
