About the Position
Goodwin Home Health is currently seeking a Weekend Home Health Intake Specialist. This position coordinates access to the Home Health program and is responsible for educating providers and the community about Home Health, as well as daily referral coordination to ensure streamlined admission to Home Health services. The Home Health intake specialist will review referrals via hospital portals, phone calls, emails, and faxes, and respond within 30 minutes. The intake specialist will input/upload all clinical records and patient information into electronic EMR, maintain system for communicating with the intake and administrative team, referring physicians, and other duties as assigned to support the mission of Home Health at Goodwin House. Hospital discharge planning, Utilization Review or Home Health knowledge is preferred. Will consider intake and admissions coordinator experience. Goodwin Home Health provides skilled care throughout Northern Virginia, focusing on the counties of: Arlington, Alexandria, Fairfax, Prince William and Loudoun.
Job Duties
- Responds to referral sources in a professional and personable manner using attention to detail while following specific protocol to input and track referrals.
- Participates in the admission process by processing and evaluating referrals, determining eligibility in collaboration with intake coordination team, and Home Health management team as needed, setting up the admission documentation in the chart, and assigning the admission/ start of care.
- Collaborates with the billing/finance team to determine eligibility and benefits for patients.
- Assists with upcoming audits with State, Medicare and ACHC agencies and completes/prepares documents required to meet regulatory requirements.
- Maintains confidentiality of patient and staff information at all times.
Job Requirements
- Two (2) years of home health experience preferred, preferably in admission or intake, and experience in working with hospital and skilled nursing case managers.
- Proficient in Microsoft Office and Excel, be willing to adapt to various EMR systems currently used as well as other programs used throughout the community.
- Effective communication, documentation, and group dynamics skills.
