ABOUT GOODWIN HOUSE HOSPICE

Help us grow a team of seasoned hospice professionals. Goodwin House Hospice is a CHAP-accredited team that promotes high professional standards in end-of-life care. We consistently have some of the highest Hospice Compare scores in the Northern Virginia area. The team encourages continuing education and self-care and supports strong team collaboration, respecting all disciplines in the goal to bring quality hospice care. 

POSITION OVERVIEW

The Goodwin House Hospice team is seeking a full-time Admissions Nurse to support a team covering the Northern Virginia area within the beltway. Work with seasoned hospice professionals in caring for patients and families in their community and facility homes at the end of life. Candidates will support the hospice interdisciplinary group (IDG) by working with referral sources to determine clinical eligibility, conduct information visits and admit eligible patients at hospitals, home or facilities.

JOB DUTIES

  • Presents hospice services and philosophy of care to patients and families faced with the challenges of arranging end of life care.
  • Performs in person assessments to determine eligibility for hospice, receives certification from the hospice physician and obtains consent for service.
  • Coordinates discharge plans and services needed for a smooth transition to home, long-term care or assisted living facility.  
  • Prepares patients and families for transition home and next steps in the receipt of hospice care.
  • Communicates with the receiving interdisciplinary team about the goals of care, initial plan of care and patient/family expectations.  
  • Completes admissions process and initiates plan of care.
  • Initiates hospice care and ensures immediate care needs are addressed.   
    Care Specific Qualities  
  • As a liaison with the community, excels and models professional and customer-service oriented values representing the mission and vision of Goodwin House. 
  • Works effectively with IDG. 
  • Demonstrates calm, efficient demeanor; is tactful and positive.
  • Excellent verbal and written communication skills.
  • Assists, resolves, or appropriately refers customer service issues.

JOB REQUIREMENTS

  • A registered nurse who is currently licensed by the Commonwealth of Virginia Department of Health Professionals and who has at least 2 years of hospice experience, preferably in liaison work.  
  • Certified Hospice and Palliative Care, preferred.
  • Demonstrated ability to assess and respond to the needs of terminally ill residents and families in varied settings.
  • Demonstrated ability to cope with family emotional stress and tolerance of individual lifestyles.
  • Demonstrated computer skills and ability to learn and use software.
