 About the Position

As a Hospice Intake Specialist, you will be a dynamic member of the Hospice Admissions team serving as a resource of information for prospective patients and a key player in coordinating hospice admissions. This position is a hybrid administrative and field position. As a member of the Hospice Admissions team, some days your responsibilities will include work such as managing referrals, verifying insurance, EMR data entry, and scheduling of admissions nurses. Other days you may spend time in the field providing informational presentations, gathering eligibility information, or completing informed consent with prospective hospice patients. This is a high-impact role requiring attention to detail, excellent communication, and strong organizational skills to ensure the high-quality, efficient, and compassionate delivery of care.

Job Duties

  • Provides accurate and timely information regarding hospice services to anyone inquiring
  • Receives calls from referral sources, responds timely, and initiates Intake process
  • Collaborates with different referral sources– SNF’s, ALF’s, Hospitals and Physicians
  • Communicate effectively and efficiently to meet the needs of patients, families, and referral sources
  • Serves as patient advocate when gathering eligibility and obtaining informed Consent for hospice
  • Develops and maintains relationships with referral sources, such as hospitals, residential facilities and physician offices to support positive patient outcomes
  • Coordinates patient admissions with the team, including obtaining all required documentation to complete referral to admission process
  • Conducts field visits as needed to provide information, help providers gather eligibility information, and identify barriers to care
  • Ensure compliance with HIPAA and agency protocols
  • Accurately input and maintain patient records in the EMR system
  • Keeps accurate records of patient and referrals source interactions.
  • Track and follow- up on referrals for patients yet to be admitted
  • Track and follow – up on unsigned CTI’s
  • Escalates customer concerns in timely manner
  • Demonstrate adaptability and readily embrace change (e.g. patient acuity, staff schedules, referral partner needs) if / when it arises. Be ready to do what’s required to best-support the patient’s goals and team needs

Job Requirements

  • High School Diploma or equivalent; Medical Assistant, or other allied health or social work professional or equivalent as demonstrated by work history
  • Minimum of 5 years of healthcare experience preferred in either a hospice hospital, Long Term Care, or Home Health setting in one of the following, or similar, roles: clinical liaison, referrals and intake coordination, discharge planner
  • Proficiency with EMR systems and referral portals such as Epic, Aiden, Point Click Care, WellSky, or similar and ability to learn new systems
  • Proficiency with Microsoft Products, Outlook, Docusign
  • Strong organizational skills and high attention to detail
  • Ability to self-direct, multitask, and prioritize in a fast-paced environment
  • Excellent verbal and written communication skills
  • Bilingual skills (e.g. Spanish, Korean) are a plus
  • Reliable Transportation
