About the Position

All employees are responsible for extending the mission and values of the organization by understanding the needs of those they serve. Each resident, staff member and community member will be treated in a dignified manner with respect, kindness, and understanding. Job Duties

  • Responsible for complying with regulations, laws, policies and procedures applicable to hospice and reimbursement issues. Educates all staff members about state and federal rules and regulations. Tracks and interprets new regulations and trains staff in their discipline-specific role in complying with these regulations.
  • Facilitates the organization's continuous state of CMS and CHAP survey readiness through activities such as but not limited to: on-site support, auditing to determine data analysis and trending, benchmark comparisons, observational visits with staff, mock surveys, participation in IDTs, providing organizational monitoring of all aspects of hospice care provided, implementing quality improvement initiatives, and participating in on-site surveys as needed.
  • Responsible for and ensures a comprehensive quality assessment and performance improvement (QAPI) program for the Hospice program. Leads the QAPI team, initiates meetings and reports outcomes.
  • Promotes positive motivation, identifies problems and/or deficiencies, assist in developing action plans, educates staff, and implements solutions for the QAPI program and follows up in a timely manner.
  • Participates with the physician, referral sources, and professional staff in decisions regarding patient eligibility and suitability for the hospice program.
  • Responsible for setting up an audit program for medical records to meet quality and regulatory clinical indicators.
  • Responds to requests for ADR’s and follows the ADR through the appeal process, if necessary
  • Coordinates employee health and education activities for hospice staff and volunteers.
  • Works with the executive director, compliance officer, medical director and/or clinical director in structuring operational or clinical processes to enhance quality patient care and to improve the work environment for staff.

Job Requirements

  • Bachelor’s degree in Nursing required.
  • Master’s degree is highly preferred.
  • Minimum 1 year of hospice experience required.
  • Must obtain Certified Hospice and Palliative Nurse (CHPN) certification within one (1) year of hire.
  • Strong clinical assessment, communication, and organizational skills.
  • Ability to collaborate effectively with interdisciplinary teams and support quality improvement initiatives.
  • Commitment to patient-centered care and adherence to hospice philosophy and standards
