About the Position

This position supports the hospice interdisciplinary group (IDG) by working with the referral team to determine clinical eligibility, admitting patients and conducting information visits at hospitals home or facilities when needed.

Job Duties

  • Acts as the clinical support on the referral team to determine eligibility for hospice and receives certification from the hospice physician and communicate clinical needs to hospice team.
  • Meets with patients and families in hospital as liaison to ensure efficient transition from hospital to home.
  • Conducts informational visits with patients and families about hospice services.
  • Helps to ensure smooth transition between referral and admission.
  • Will admit patients to hospice as needed.

Job Requirements

  • Graduate of an accredited school of nursing
  • Licensed to work as a Registered Nurse in the Commonwealth of Virginia
  • At least on year acute care experience, Hospice preferred
  • Current CPR certification
  • Knowledge of the duties and functions of Home Health and Hospice care and its relationship to the healthcare community is preferred.
  • Must know or willingness to learn and perform HIS documentation and SOC.
  • Must possess the ability to make independent decisions and work independently thought out the workday.
  • Must possess a driver’s license, proof of automobile insurance and readily available, reliable transportation.
  • Must possess the ability to deal tactfully and compassionately with patients, family members and caregivers.
  • Must be willing to and able to participate in a teamwork approach, working with all disciplines involved in the patient’s care/case.
